Chronic obstructive pulmonary disease (COPD) is\na severe chronic disease that needs continuous care from the time of hospital\nadmission extending to long-term ongoing self management. <\/p>\n\n\n\n<p>The current review highlights the benefits\nof digital health interventions in COPD care. It also discussed the gaps and\nthe need for further studies in COPD care.<\/p>\n\n\n\n<p>The review is based on several recent\nand emerging studies along the pathway of COPD management from in-hospital\ncare, post-discharge care, hospital-at-home, self management. The review also\nconsidered several studies on the impacts of environmental factors and public\nhealth surveillance, where environmental sensors, satellite images, wearable\ndevices and social media were used. &nbsp;<\/p>\n\n\n\n<p>Several studies have demonstrated that\ndigital applications can potentially reduce hospitalizations and mortality in\nCOPD patients. Some of the studies demonstrated positive effects on reducing\nhospitalizations by 20% with the use of telemonitoring. Implementation of these\napplications is often found difficult. This demands the need for\npersonalization of the service that increases its adaptability both by patients\nand healthcare professionals. Digital transformation enabled population\nstudies also helps in improving public health surveillance.<\/p>\n\n\n\n<p>The\nuse of AI in the analysis of personalised data that is collected via smartphone\nsensors and wearable devices is also the current topic of research. It provides\nnew opportunities for early prediction and prevention of COPD exacerbation. <\/p>\n\n\n\n<p>However, clinical evidence for the\nimpact of digital transformation in COPD care, especially for in-hospital and\npost-discharge care is limited.
